Patients who are not appropriate for transfer to Amery Hospital include those requiring:
- Burn Care
- Cardiac catheterization
- Critical Care
- Hemodialysis
- Interventional radiology
- Pediatrics
How does the patient transfer process work?
HealthPartners Direct facilitates hospital admissions and emergency department transfers. When you call, a staff member will ask a few questions about the patient and referring provider, then connect you with a member of our care team. If necessary, we’ll arrange a safe and timely patient transfer.
Necessary patient transfer information
To connect the referring provider to the appropriate services, we’ll need the following information. Please have this information ready when you call HealthPartners Direct. (Note: We may ask additional triage questions, depending on the patient’s diagnosis.)
- Caller name
- Callback phone numbers (including direct number to referring provider, if different than the caller)
- Provider first and last name
- Patient demographics (including first name, last name, date of birth and medical record number)
- Diagnosis and reason for transfer
- Isolation precautions
- IV drip medications (other than saline)
