Back-to-school Well Exams & Sports Physicals

A well-child visit covers all your child’s back-to-school health care needs. Unlike retail clinics, we offer comprehensive care that focuses on your child’s complete health – physical, developmental and behavioral. Depending on your child’s age, visits include a check of height and weight, body mass index (BMI), developmental milestones, vision, hearing and blood pressure. We’ll also review and update immunizations – including annual flu shots – and discuss safety habits.

For kids in school sports, a well-child visit can also include sports clearance. Bring any sports or school-required forms to your appointment and we’ll help you complete them. Download Wisconsin Interscholastic Athletic forms or pick them up at one of our locations. We offer appointments for kids in Pediatrics and Family Medicine. Schedule an appointment at clinic near you!

Please bring your completed athletic form signed by a parent/guardian. Parents/guardians are strongly encouraged to accompany athletes to their appointments.
