A patient is considered eligible for ARMC financial assistance, if they fall within the following guidelines:
1. Patients applying for ARMC Assistance must exhaust all governmental assistance programs first, such as Medical Assistance, Badger Care, and General Relief.
2. Gross income for your household cannot exceed the Department of Health and Human Services guidelines. ARMC assistance guidelines are based on up to 200% of the Federal Poverty Guideline level and are reviewed annually.
3. Assets and Liabilities are factored into the application review, utilizing home and land equity, all types of vehicles, as well as cash and investment assets.
4. You will need to submit an ARMC Application for Assistance and include the following information below. Please carefully read the ARMC Application for Assistance Instructions as this document thoroughly explains documentations and approvals needed for assistance.
(a.) A complete copy of your most recent income tax return. Attach proof of income for each source of income listed in the Income Section of the application. (Examples: last two check stubs from employer, court ordered child support letter, annual Social Security letter indicating monthly benefit amount) Please include copies, not originals.
(b.) Copies of your last three bank statements.
(c.) A copy of your most recent property tax statement for any property owned.
5. If determined eligible, the balance you are responsible for must be paid within the ARMC payment plan guidelines. You will receive communication in writing of program approval and associated assistance or program denial.
6. With the exception of emergency services, persons residing in the ARMC primary service area will be given preference in the ARMC Assistance application approval.
