Basic Benefits Program

Note: Unless other requirements are otherwise stated – All full time and part time employees become eligible for benefits on the first day of the month following 90 days of continuous service.

Group Health Insurance
This plan is designed for employees and their families to protect against catastrophic health expenses as well as encourage them to take preventive measures to ensure better health.

ARMC and the employee will both participate in the cost for the coverage. Employee contributions are deducted pre-tax, twice monthly.

Health Savings Account
If you participate in ARMC’s group high deductible health plan (HDHP), you will have the opportunity to contribution pre-tax dollars to a Health Savings Account. These dollars can be used to pay your health insurance deductible as well as vision and dental care expenses. This differs from a Flexible Spending Account in that you do not ever lose the dollars you put in the account. ARMC will also match a portion of your contributions each year.

Group Dental Plan
Our dental coverage includes Diagnostic and Preventative, Basic, Major and Orthodontia services. As in the health plan, ARMC and the employee both participate in the cost for coverage.

Benefit Waiver
Employees who elect to waive health and/or dental insurance coverage will have an amount added to their paycheck twice monthly.

Wellness Initiative
ARMC has instituted a very innovative wellness program. Voluntarily participate in offered wellness initiatives and receive a cash initiative to your paycheck twice monthly.

Life Insurance/Accidental Death and Dismemberment (AD&D) Plan
To help ensure the financial security of those who depend upon you, we purchase coverage in the amount of $20,000 or one time your annual salary, whichever is greater, for you. You have the option to purchase additional coverage for yourself and your dependents.

Short Term Disability (STD)
As a means of providing you and your family with some income in case of your inability to work because of serious illness or accidental injury, ARMC contributes a flat dollar amount and the employee has the option of purchasing additional coverage. The plan pays up to 66 2/3% of your income and begins on the 15th day after the injury or illness. The plan continues until Long Term Disability begins, if necessary.

Long Term Disability (LTD)
LTD coverage is purchased for you by the Medical Center and payment is the same as STD (66 2/3% of base income). This coverage begins after 180 days and continues and coincides with Social Security, if necessary.

Group Critical Illness
Offered once each year, employees have the opportunity to purchase this insurance and be paid a lump sum benefit in cash at the diagnosis of a covered illness to be used for unforeseen expenses not covered by health insurance.

Accident Insurance
Offered once each year, employees have the opportunity to purchase this insurance which provides a lump sum benefit based on the type of injury or covered incident sustained. These dollars can be used for unforeseen expenses not covered by health insurance.

Direct Deposit
Paychecks can, and are encouraged to be directly deposited each pay day, into one or several of your personal accounts for your convenience.

Retirement Plan with Employer Matching Dollars
If you are a full-time or part-time employee, you may begin to set aside a portion of your gross pay each pay period in convenient pre-tax or after-tax (Roth) dollars as soon as you begin employment. After one year of continuous service, and if you contribute at least 2%, ARMC will also match that amount. The plan gives you investment options so you can select the investment strategy that makes the most sense for you and the flexibility to change how you save and invest that money.

Workers’ Compensation Plan
This coverage is for 66 2/3% of salary if an employee is injured or becomes disabled due to their position with the Medical Center. The premium is paid by ARMC.

Social Security
Contributions to Social Security are paid by both Amery Regional Medical Center and you.

Flexible Spending Account (Section 125)
This is a pre-tax account available to those of you who pay day care, or have additional health and dental expenses that are not covered under the health and dental plans. It is processed directly through our payroll system for efficiency.

Paid Time Off (PTO)
Rather than providing employees a specific number of vacation days, holidays and sick days, Amery Regional Medical Center uses an innovative, all encompassing Paid Off Time System. This approach provides for the creation of a PTO account for each eligible employee into which the Medical Center places accrual hours, according to a worked hour formula, each pay period. Payment for any time off is made to you from your PTO account.

Tuition Assistance
We believe continuing education is important and this benefit is designed to accommodate employees who want to go back to school and obtain additional academic credentials.

Other Educational Opportunities
Seminars and other such educational activities are judged on relativity to the work you do, and portions or all the this education may be paid by ARMC.

Shift Differentials
Shift differentials are paid for evenings and nights according to job classifications, and worked holidays are paid at 1½ times your hourly rate.
