Open Admissions Policy
It is the policy of Amery Regional Medical Center, Amery WI, to admit and treat all persons without regard to race, color, sex, handicap, national origin or religious creed. Admission requirements and assignments of hospital facilities are the same for all persons. There are no distinctions in eligibility for receiving any patient care services. Hospital facilities are available to all patients and visitors. Individuals and organizations having occasion to refer patients for admission or recommend Amery Regional Medical Center are advised to do so within the hospital’s policy to provide quality healthcare to all persons.

Your Admission
Registration is located in the main lobby of the Medical Center. Patients or their immediate family members are requested to provide insurance & information concerning hospitalization at the time of admission.

If you have a pre-scheduled appointment for registration, it is important that you arrive on time, because your physician may have scheduled special tests for you (i.e., blood, x-rays, etc.) You will be asked to sign a consent form for admission to the hospital and for medical treatment. An identification bracelet for your wrist will also be prepared.

At this time you will be taken to the nursing unit to which you are assigned. A nurse will interview you by asking questions about your general condition, diet, allergies, personal habits and other factors which will apply specifically to the nursing care you will receive. Federal regulations require hospital personnel to inquire as to the presence of an Advance Directive. This includes a living will or durable power of attorney with a healthier clause. Information on Advance Directives is provided to every patient upon admission. Please feel free to ask the nursing staff any questions you have about our hospital, staff or procedures. We will try to provide you with the room accommodations of your choice. However, on occasion, the accommodations you prefer might not be immediately available because of physician requests, special treatment procedures or the medical needs of other hospitalized patients.

CRITICAL ACCESS DESIGNATION... Amery Regional Medical Center is surveyed by the State of Wisconsin on a regular basis and is a Critical Access Hospital. This designation is important for the continued financial viability of our Medical Center. Accreditation through the State of Wisconsin means they review our facility and healthcare operation; this is your assurance that Amery Regional Medical Center practices continuous quality monitoring to provide you with superior health care.

Patient Release of Information
If you would like us to release your medical information to another facility (insurance, other medical, legal, etc.), please fill out the Patient Authorization For Release of Information. If you are a patient at Amery Regional Medical Center or Clear Lake Clinic, please fax the form to our Medical Records department at (715) 268-0381. If you are a patient at Luck Medical Clinic, please fax the form to (715) 472-8787.


Patient Account Information/Insurance Plans
Amery Regional Medical Center and its affiliates accept a variety of insurance plans and network providers.

The following is a list of some of the major carriers: Anthem BCBS, BCBS-MN, Group Health or CompCare: Managed Medicaid HMOs, Health EOS, HealthPartners ,HIRSP, Medica, Medicaid-Wisconsin, Medicare, Preferred One, Select Care, Tricare/ChampVA, UCare (Medicare Advantage), WEA, Wisconsin Well Woman Program, WPPN, and WPS.
